Job summary

Community Housing Innovations (CHI) is hiring for a Full Time position located in Coram, NY. The role involves helping clients develop Independent Living Plans, providing counseling, referrals and direct services to support housing stability.

Requirements include a Bachelor’s in human services or related field, with a Master’s in Social Work preferred; English proficiency is required and Spanish is a plus. Experience in homelessness or housing is preferred.

Qualifications

• Bachelor's degree in human services or related field, Master’s preferred.
• Two years’ experience working in homelessness or housing.
• Proficiency in English required; bilingual in Spanish is a plus.

Responsibilities

• Adhere to and enforce program regulations, policies, and procedures; ensure clients receive quality services.
• Assist clients in developing an Independent Living Plan (ILP) by identifying needs and goals; evaluate progress.
• Provide information, referrals, counseling, crisis intervention, direct service, and advocacy for all clients.
• Develop and maintain a resource network for referrals; collaborate with DSS and other service providers.
• Assist clients with direct services such as budgeting and resume preparation; support ADL skills.
• Conduct face-to-face meetings with clients and inspect living space for program compliance.
• Limit and document improper use of agency resources; report needs to supervisor.
• Submit internal and external reports timely; maintain charts and files.
