Qualifications

• Education: Bachelor’s degree in social work or psychology preferred.
• Two years’ experience in case management, counseling, or direct advocacy for clients in social service settings.
• Ability to coordinate services, communicate clearly, and maintain documentation.

Responsibilities

• Coordinate service and care plans, provide counseling and direct services.
• Develop discharge plans and ensure enrollment in public benefits and DHS programs.
• Meet residents weekly to assess progress and modify plans as needed.
• Document interactions in CARES and AWARDS and coordinate with external providers.
