Job summary

Community Housing Innovations (CHI) is seeking a full-time Case Manager for our Riverhead location to ensure participants receive services that promote personal and economic independence, help them obtain and maintain permanent housing, and live in well-maintained units.

The ideal candidate has a Bachelor's degree in human services (Master's in Social Work preferred), at least two years' experience in homelessness or housing, excellent verbal and written communication, English proficiency

Qualifications

• Bachelor's degree in human services or related field required.
• Two years' experience in homelessness or housing preferred.
• Excellent verbal and written communication skills; English required; bilingual Spanish a plus.

Responsibilities

• Adhere to and enforce program regulations, policies, and procedures; ensure clients receive quality services.
• Assist clients in developing an Independent Living Plan (ILP) and evaluate progress.
• Provide information, referrals, counseling, crisis intervention, direct service, and advocacy.
• Develop and maintain a referral network; collaborate with DSS and other service providers.
• Assist clients with budgets, resumes, ADL skills, etc.
• Face-to-face meetings with clients.
• Inspect living space to assess needs and compliance.
• Limit improper use of agency resources; document issues.
• Evaluate and report housing unit needs to supervisor.
• Complete discharge plans with clients.
• Submit all reports to funding agencies and CHI.
