CHI in Brooklyn, NY is seeking a Case Manager to develop and execute service and care plans for residents, coordinating direct services and referrals to external providers to promote stabilization and independence.

The role requires strong communication, experience in social services, and ability to engage with diverse communities; schedule includes some weekend hours and non-exempt status.

Qualifications

• Education: HS diploma or GED required; BA preferred.
• Two years’ experience in case management, counseling, or direct advocacy for clients in social service settings.
• English proficiency required; multilingual and Spanish proficiency a plus.

Responsibilities

• Coordinate service and care plans for residents with the care team.
• Link residents to external and community services and providers.
• Meet with residents weekly to assess progress and update plans.
• Coordinate with social workers to monitor health, safety and behaviors.
• Develop discharge plans to maintain benefits and enrollment.
