Job summary

Qualifications

• Minimum two years of experience working with low-income or culturally diverse populations.
• Experience providing housing stabilization or case management services.
• Familiarity with Housing First, Harm Reduction, and Trauma-Informed Care frameworks.
• Knowledge of community-based resources that support housing stability, including rental assistance programs.
• Experience with eligibility verification, program enrollment, and financial assistance tracking.
• Experience with landlord engagement, mediation, or tenant advocacy preferred.

Responsibilities

• Manage a caseload of approximately 20–25 participants, providing stabilization services.
• Conduct regular meetings with participants and adjust frequency/location based on need.
• Maintain accurate, timely documentation in HMIS.
• Provide ongoing needs assessments and monitor progress toward housing stability goals.
• Administer and track financial assistance in accordance with program guidelines and approvals.
• Facilitate matches for high-acuity participants to Permanent Supportive Housing and support transitions.
• Coordinate with Housing Services Manager for rental approvals and case consultations.
• Collaborate with internal teams and external service providers to ensure coordinated care.
