Job summary

Hope the Mission is seeking a Housing Navigator (FT & PT) to help move people from homelessness into permanent housing. This field-based role collaborates with participants, landlords, and service providers to assess needs, develop individualized housing plans, and guide clients through applications, vouchers, move-in, and stabilization.

Reporting to the Program Manager, you will work across LA programs (Section 8, VASH, DHS Housing for Health) and maintain HMIS documentation, meet performance

Qualifications

• Bachelor's degree in Social Work or related field or equivalent experience.
• Minimum 3 years of experience with homelessness or related populations.
• Knowledge of Housing First, Harm Reduction, Trauma-Informed Care; HUD housing programs familiarity.

Responsibilities

• Engage participants in housing-focused case management and build trusting relationships.
• Conduct assessments to determine appropriate permanent housing interventions.
• Provide housing readiness support including documentation, applications, and eligibility.
• Monitor progress toward housing goals through regular follow-up.
• Assist with voucher applications through LAHSA and related programs.
• Identify best-fit placement options based on participant needs.
• Coordinate move-in assistance resources and housing supports.
• Maintain HMIS data and ensure documentation meets funder requirements.
