Job summary

Pacific Health Group is seeking a field-based Housing Stability Services professional to support Medi-Cal members with housing transitions, tenancy services, and coordination of healthcare and community resources. The role combines in-field visits with telephonic outreach to ensure timely housing access and ongoing support.

You will manage a high caseload, document in real time, and collaborate with housing partners, shelters, and healthcare providers to sustain housing stability for high-need

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Ability to manage an active caseload independently.

Responsibilities

• Provide housing stability services and care coordination.
• Coordinate housing deposits and tenancy services.
• Engage with housing authorities, landlords, shelters, and community organizations.
• Monitor housing stability and address barriers to care.
• Maintain timely documentation and records of member interactions.
• Collaborate with interdisciplinary teams to ensure seamless care delivery.
