Job summary

Pacific Health Group seeks a field-based Housing Stability Specialist to support CalAIM Community Supports housing programs. You will conduct intakes, build housing care plans, and coordinate deposits and tenancy services, traveling across counties.

The role requires independent field work, strong client engagement, and real-time documentation. Ideal candidates have 3–5 years in care coordination or social services, familiarity with Medi-Cal/CalAIM, and excellent communication and organizational

Qualifications

• 3-5 years in case management, social services, or healthcare
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred)
• Experience independently managing an active caseload in a care coordination, housing navigation, or case management environment
• Strong understanding of healthcare systems, managed care, and community resources
• Excellent communication, empathy, and cultural competence
• Strong organizational and time management skills
• Proficient with technology, including case management systems, EHR platforms, and documentation tools
• Ability to document in real-time and manage member data accurately

Responsibilities

• Provide Housing Transition Navigation Services and Housing Tenancy & Sustaining Services
• Coordinate Housing Deposits and documentation to facilitate access to deposit assistance
• Collaborate with housing authorities, shelters, landlords, and providers to ensure successful housing placement
• Travel locally within the county and surrounding counties for field-based visits
• Maintain high-volume outreach, initial intake assessments, and ongoing care plan development
• Maintain real-time, compliant documentation and reporting of member interactions
• Monitor and support 60-70 high-needs members through end-to-end care coordination and housing stabilization milestones
