Job summary

Pacific Health Group seeks a field-based Housing Navigator to support Medi-Cal Community Supports in Mendocino County. You will assess housing needs, coordinate deposits, and connect members with housing resources while partnering with housing authorities, shelters, and providers.

This role requires independence, strong time management, and the ability to travel locally within the county. You will conduct in-person visits, document real-time notes, and advocate for timely housing services to

Qualifications

• Strong understanding of housing navigation and case management in community settings.
• Experience with Medi-Cal, CalAIM, and community supports programs is preferred.
• Ability to document in real-time and manage member data accurately.

Responsibilities

• Provide Housing Transition Navigation Services and Housing Tenancy & Sustaining Services.
• Coordinate deposits and housing resource documentation.
• Build partnerships with housing providers, shelters, landlords, and community organizations.
• Conduct home visits, assessments, and follow-ups in the field.
