Job summary

Pacific Health Group in California is seeking a field-based Housing Stability Navigator to support Medi-Cal members with comprehensive housing-focused care management, including housing deposits, tenancy services, and access to community resources.

You will split time between in-person field visits and telephonic outreach, building partnerships with shelters, landlords, and housing authorities to secure stable housing and ongoing health supports.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Ability to manage an active caseload independently in care coordination or housing/navigation.
• Strong organizational and time management skills.
• Proficient with case management systems, EHR platforms, and documentation tools.
• Excellent communication, empathy, and cultural competence.

Responsibilities

• Provide housing care management and coordinate housing resources.
• Travel locally within the county and surrounding counties.
• Engage with housing providers, shelters, landlords, and community organizations.
• Maintain real-time documentation per state guidelines.
• Support field-based work delivering face-to-face case management.
• Build referral routes and community partnerships.
