Job summary

Pacific Health Group is seeking a field-based housing stability specialist to support Medi-Cal members with Housing Transition Navigation, Housing Tenancy, and Deposits services across Mendocino County and surrounding areas.

You will conduct intake assessments, coordinate housing resources, and build strong community partnerships to improve housing access and long-term stability. Travel locally is required and documentation must be accurate and timely.

Qualifications

• 3-5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Strong understanding of healthcare systems, managed care, and community resources.
• Excellent communication, empathy, and cultural competence.

Responsibilities

• Provide Housing Stability Services, including Housing Transition Navigation and Housing Tenancy & Sustaining Services.
• Coordinate Housing Deposits and documentation to access deposit assistance.
• Collaborate with housing authorities, landlords, shelters, and providers to ensure housing placement.
• Conduct field visits and home inspections; maintain real-time documentation of each case.
• Build partnerships with community organizations to expand housing access and resources.
• Represent Pacific Health Group in community outreach and events to drive referrals.
• Manage and coordinate the care pipeline for housing resources and essential documents.
