Pacific Health Group is seeking a field-based Housing Navigator to support CalAIM Community Supports Housing programs and help members access stable, long-term housing. You will conduct intake assessments, coordinate housing resources, and collaborate with landlords, shelters, and healthcare providers to ensure access to services.

You will spend about 65% of time in the field, travel locally within the county, and maintain real-time chart notes following state guidelines while advocating for

Qualificações

• Strong understanding of housing system and local housing resources/workflows
• 3-5 years in case management, social services, or healthcare
• Experience with Medi-Cal, CalAIM, and Community Supports (preferred)
• Strong understanding of healthcare systems, managed care, and community resources
• Excellent communication, empathy, and cultural competence
• Strong organizational and time management skills
• Proficiency in technology, including case management systems and documentation tools
• Ability to document in real-time and manage member data accurately
• Comfort navigating multiple systems and communication platforms

Responsabilidades

• Support members in CalAIM Housing programs and housing deposit assistance
• Conduct intake assessments and build housing partnerships
• Provide housing care management and coordinate with housing providers
• Spend significant time in the field (home inspections, shelters, community settings)
• Travel locally within the county for in-person visits
• Coordinate housing resources documentation and landlord allocations
• Engage with community outreach and partnerships to expand referrals
• Maintain accurate, real-time notes and documentation per state guidelines
• Operate independently in field and remote environments
