Job summary

The Salvation Army USA Western Territory is seeking a Housing Navigator/Case Manager to screen applicants, process admissions and referrals, and help residents access benefits and housing resources. You will collaborate with housing providers to maintain an accurate referral list and support residents toward stable housing.

The role requires experience in case management, familiarity with HMIS, and knowledge of mental health and trauma-informed care.

Qualifications

  • Associate degree in social work, human services, or related field. Related experience may be considered in lieu of degree.
  • HMIS or other information management system experience preferred.
  • Working knowledge of mental health, substance abuse, and dual diagnosis treatment philosophies.

Responsibilities

  • Screen and update list of those seeking services and provide necessary referrals.
  • Case manage clients and families; complete intake, assessment, and individual case plans.
  • Maintain resident information in software; manage rosters and case records accurately.
  • Provide referrals to benefits, programs, and resources to support stability.
  • Coordinate with community partners and service providers to aid housing opportunities.
  • Maintain confidentiality and adhere to social service ethics.
