Job summary

Hope Haven in Charlotte, NC is seeking a dedicated housing specialist to support residents in obtaining and maintaining stable housing. You will perform comprehensive housing assessments, assist with searches and eligibility, and connect clients to local resources and TBRA programs.

Responsibilities include ongoing case management, advocacy, documentation in EHR systems, ensuring HIPAA compliance, and collaborating with community partners.

Qualifications

• Minimum of a high school diploma or GED with at least three years in a related field.
• Demonstrated emotional stability and maturity to engage with a diverse, underserved population.
• Excellent verbal and written communication skills.
• Familiarity with community housing resources.
• Valid North Carolina driver’s license and insurability.
• Willingness to provide direct support within the facility on a 24/7 basis as needed.
• Knowledge of confidentiality laws governing client information.

Responsibilities

• Conduct housing assessments and develop individualized housing stability plans.
• Assist residents with housing searches, applications, placement activities, and eligibility documentation.
• Research and connect residents to affordable housing options and TBRA resources.
• Provide ongoing case management, advocacy, and resource coordination.
• Identify barriers to housing stability and address resident needs.
• Empower residents and monitor progress toward housing goals.
• Support management of independent housing residents, including inspections and lease requirements.
• Maintain knowledge of TBRA and other housing programs.
• Document all resident interactions in EHR or designated systems.
• Ensure HIPAA compliance and confidentiality of resident information.
• Represent Hope Haven at housing coalitions and community events.
• Participate in staff meetings and professional development.
• Other duties as assigned.
