Job summary

The Salvation Army Southern California is seeking a Housing Navigator/Case Manager to support residents by screening, assisting with benefits applications, and coordinating housing referrals. You will work under the Director for Programs to help residents access resources and achieve stable housing.

Responsibilities include maintaining participant data, collaborating with community partners, and coordinating with service providers to ensure timely support and documentation.

Qualifications

• Associate degree in social work or related field or two years’ experience with similar populations.
• HMIS or equivalent information management system experience preferred.
• Working knowledge of mental health, substance use, and dual diagnosis treatment philosophies.

Responsibilities

• Screen and update list of those seeking services and provide referrals.
• Case manage clients and families with intake, assessment, and action plans.
• Maintain resident information and case records in systems and rosters.
