Job summary

St. Francis House in Boston seeks a Housing Stabilization Case Manager to help residents move from homelessness to permanent housing. Working from a Housing First approach, the HSCM supports skill-building and access to resources for long-term housing stability.

Responsibilities include managing a caseload of ~15 residents, conducting assessments, developing service plans, and coordinating with partners to address housing, income, and behavioral health needs.

Qualifications

• Bachelor’s degree in Social Work or related field; 3+ years preferred.
• High School diploma or GED with 10+ years experience considered.
• Experience in direct service or case management with vulnerable populations.

Responsibilities

• Welcome residents and support transition from homelessness to housing.
• Manage a caseload of ~15 residents, ~30% high-level support.
• Build relationships through outreach to connect residents with services.
• Conduct assessments and develop service plans for housing, income, health.
• Support daily living skills like budgeting, tenancy, nutrition.
• Collaborate with team and partners to plan activities and support.
