Job summary

Action Inc. is seeking a Housing First Case Manager to provide home and community-based case management for individuals and families experiencing chronic homelessness. The role embraces the Housing First model, harm reduction, and helping clients secure stable tenancies.

The ideal candidate will hold a BS in Human Services or have 3 years related work, possess a valid driver's license, and be able to lift 25 lbs. CORI checks and E-Verify employment are part of the process.

Qualifications

• BS degree in Human Services or related field or 3 years relevant work experience.
• Valid driver's license and reliable transportation.
• CORI background check and E-Verify eligibility.

Responsibilities

• Provide case management services with home visits, phone and office contacts.
• Assess client needs and coordinate with health and housing resources.
• Develop and modify service plans quarterly.
• Work with landlords to support tenancies.
• Maintain accurate records in agency databases.
• Uphold client confidentiality and professional boundaries.
