Job summary

Model Cities of St. Paul, Inc. seeks a Housing Case Manager to help individuals and families obtain and maintain safe, stable, affordable housing.

You will provide case management, housing navigation, advocacy, and resource coordination to support housing stability and long-term self-sufficiency. The ideal candidate is compassionate, organized, resourceful, and committed to trauma-informed and culturally responsive services.

Qualifications

• Associate's or Bachelor's degree in Human Services, Social Work, Community Development, or a related field, or an equivalent combination of education and experience.
• One to three years of experience in case management, housing services, or related human services work.
• Strong communication, organization, and relationship-building skills.
• Proficiency with Microsoft Office and data management systems.

Responsibilities

• Case Management & Housing Support: Assess participant needs and develop individualized housing stability plans; Provide ongoing case management, coaching, advocacy, and follow-up; Assist participants with housing searches, applications, move-ins, and housing retention.
