St. Francis House in Boston, MA is hiring a Residential Services Case Manager to provide direct support to individuals transitioning from homelessness to permanent housing.

The role follows a Housing First, trauma-informed philosophy to help residents achieve housing stability, income growth, and behavioral health improvements. The position requires Motivational Interviewing, strong communication, and collaboration with community partners.

Qualifications

• High School Diploma or equivalent required.
• Experience with direct service or case management with vulnerable populations preferred.
• bilingual in Spanish preferred.

Responsabilités

• Welcome new residents and support transition from homelessness to stable housing.
• Manage a caseload of ~30 residents with ~30% needing high-level support.
• Build proactive relationships to connect residents with services.
• Develop service plans focused on housing, income, and behavioral health stability.
• Support development of daily living skills.
