Job summary

Housing Forward is seeking a Housing Case Manager to coordinate direct case management, housing navigation, and stabilization services for Families in Transition programs. The role supports clients in securing permanent housing, maintaining leases, and connecting to medical and social services.

Responsibilities include maintaining documentation in HMIS and Salesforce, developing service plans, and coordinating transportation and life-skills training.

Qualifications

• Bachelor’s degree in social service or related field; Master’s preferred.
• Two years of experience; bilingual preferred.

Responsibilities

• Coordinate direct case management, housing navigation, and stabilization services.
• Maintain HMIS and Salesforce documentation and reports.
• Develop and monitor service plans per agency standards and funding.
• Assist clients in obtaining permanent housing and addressing barriers.
• Coordinate transportation to housing, medical, and legal appointments.
• Connect clients with medical, mental health, and other support services.
• Engage in life-skills training to improve independence.
• Collaborate with landlords and service providers.
