Qualifications

• Bachelor's degree in Human Services or related field is required.
• 1–3+ years of case management or homelessness services experience preferred.
• Familiarity with Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Valid driver’s license and ability to travel locally in the Greater Boston area.
• Strong communication, organizational, and computer skills.
• Comfortable working with diverse individuals with complex service needs.

Responsibilities

• Conduct outreach, intake, assessments, and eligibility screenings for housing support.
• Help participants identify housing options and develop plans for stable housing.
• Provide ongoing case management and connect to employment, legal, behavioral health, benefits, and community resources.
• Assist with addressing barriers to housing, including credit, rental history, and legal concerns.
• Coordinate housing placement, homelessness prevention, and stabilization services within program timelines.
• Build and maintain relationships with landlords, housing resources, and community partners.
• Maintain accurate case documentation, data entry, and reporting; participate in meetings.
