Pacific Health Group is seeking a field-based Housing Stability Navigator to support members experiencing homelessness or at risk of homelessness. You will coordinate housing-related services, conduct in-person assessments, and collaborate with housing authorities, providers, and community partners.

The role requires 60–70% field time in shelters and community settings, managing a dynamic caseload with attention to documentation and regulatory compliance.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Ability to document in real-time and maintain accurate records.

• Conduct field-based housing care coordination across communities.
