Pacific Health Group in California is seeking a Housing Stability Services professional to support community members in housing transition and tenancy services. This field-based role emphasizes outreach, assessment, and collaboration with housing authorities, shelters, and healthcare partners to ensure stable housing and access to services.

You will conduct in-person visits, manage care plans, and document interactions in real-time while traveling up to 60–70% within the county.

Formación

• 3–5 years in case management, healthcare, or social services.
• Strong communication, empathy, and cultural competence.
• Ability to travel up to 60–70% within the county.
• Proficient with case management technology and documentation.

Responsabilidades

• Support members experiencing homelessness or at risk of homelessness by providing Housing Transition Navigation Services.
• Coordinate Housing Tenancy & Sustaining Services by helping members keep housing through case management and landlord engagement.
• Assist members with Housing Deposits and coordinating required documentation.
• Collaborate with housing authorities, shelters, and community partners to ensure housing placement.
• Monitor housing stability and address barriers to care.
