Pacific Health Group is hiring for a field-based Housing Stability Navigator in Santa Cruz County, CA. You will support Housing Transition Navigation Services, Housing Tenancy & Sustaining Services, and Housing Deposits while coordinating with landlords, shelters, and healthcare providers.

The role emphasizes independent work in the community, strong time management, and excellent documentation to ensure housing stability for high-needs members.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Strong understanding of housing systems and local resources.
• Proficient with case management systems and real-time documentation.

Responsibilities

• Support housing programs and housing transition services in the field.
• Coordinate deposits, tenancy and sustaining services, and landlord engagement.
• Develop care plans and coordinate healthcare, behavioral health, and social resources.
• Travel locally within county for in-person visits and assessments.
• Document interactions in real time and maintain accurate records.
