Pacific Health Group is seeking a field-based Housing Stability Specialist in California. You will support Medi-Cal members with Housing Transition Navigation, Tenant & Sustaining Services, and Housing Deposits, coordinating with housing providers and healthcare partners.

Qualifikationen

• Strong understanding of the housing system and local housing resources, housing navigation, and community-based support workflows.
• Experience in case management within health or housing programs (3–5 years).
• Proficiency with Medi-Cal, CalAIM, and Community Supports programs (preferred).

• Provide field-based housing stability services in housing communities, shelters, and residences.
• Coordinate Housing Deposits and related documentation to secure permanent housing.
• Develop and manage individualized housing care plans and ongoing support.
• Engage with housing authorities, landlords, and community organizations to enable stable tenancy.
• Maintain high-volume outreach, intake assessments, and real-time documentation.
