Upper Valley Haven is seeking a compassionate Service Coordinator to support individuals and families staying in local hotels as part of Vermont’s Homelessness Response Continuum. The role focuses on individualized, strengths-based case management with a small caseload and weekly client contact.

The position is based at the Leahy Shelter and Resource Center, with local hotel-site work and short travel in White River Junction and Bradford.

Competenze

• Bachelor’s degree in social work, human services, psychology, or a related field.
• Master’s degree preferred; relevant certification in social work, mental health, crisis intervention is a plus.
• Experience in social services, nonprofit/homeless services settings is desirable.

Mansioni

• Provide consistent, strengths-based, trauma-informed services to individuals and families.
• Build trusting relationships with clients and maintain weekly contact while managing a caseload.
• Complete Coordinated Entry assessments.
• Help clients identify housing opportunities and complete housing applications.
• Connect clients with resources for mental health, substance use, employment, and primary care.
• Assist clients in accessing public benefits (e.g., 3SquaresVT, Medicaid).
• Support clients in pursuing individualized housing goals within complex service systems.
• Apply advocacy, de-escalation, crisis management, and problem-solving as needed.
• Build collaborative relationships with community partners and agencies.
• Maintain thorough, timely, and professional documentation.
