Community Care Network in Rutland, VT is seeking a Developmental Services Housing Coordinator to create safe, person-centered housing options for individuals served. This role acts as the housing referrals hub, coordinating with local agencies, landlords and partners to expand opportunities and reduce barriers.

You will support housing searches, assist with applications, track referrals and outcomes, and participate in interdisciplinary meetings to promote residential stability and community

Formación

• High School Diploma or GED required; Associate's or Bachelor's preferred.
• Experience in developmental disabilities, housing, or case management preferred.
• Strong relationship-building and community outreach skills.
• Strong organizational, communication, problem-solving, and follow-through skills.
• Ability to manage multiple housing referrals, provider applications, deadlines and priorities.
• Proficiency with computers, documentation systems and related technology.
• Valid driver's license, reliable transportation and background checks.

Responsabilidades

• Coordinate housing referrals with local agencies and landlords.
• Develop and maintain landlord and housing partnerships.
• Assist individuals with housing searches and required documentation.
• Track housing referrals, outcomes, landlord contacts and vacancies.
