Job summary

Catholic Community Services in Washington state seeks a Case Manager for the Veterans Services program to support nine counties. You will conduct outreach, complete intakes, screen eligibility, and coordinate housing stability plans to help veterans access permanent housing.

Qualifications

• Post-secondary education in social services or 2 years relevant experience.
• Experience providing services to low-income/no-income households.
• Experience in case management.
• Ability to work in a team and promote a positive team spirit.
• Excellent interpersonal, oral and written communication skills.
• Excellent organizational and time management skills.
• Must have reliable transportation, valid driver's license.
• Background checks, TB test, CPR/First Aid training required.
• Support CCS mission and values in daily work.
• Bilingual skills (English-Spanish) preferred.

Responsibilities

• Complete thorough intakes and eligibility screening with referred clients.
• Initiate Housing Stability Plan and identify top housing needs.
• Provide outreach to Veterans and foster community referrals.
• Enter initial information into HMIS and maintain service records.
• Update Housing Stability Plan and coordinate actions across domains.
• Attend meetings and training; share information with staff.
• Perform other duties as assigned.
