Job summary

Jericho Project is seeking a Case Manager in the Bronx to provide residential and long-term support to formerly homeless and low-income adults transitioning to permanent housing.

The role carries a caseload of 20–25 participants, with regular on-site and field visits, weekly or monthly meetings, and ongoing documentation. The compensation ranges from $53,000 to $55,000 annually, with non-exempt status and a Tuesday–Saturday schedule.

Qualifications

• Bachelor's degree in human services or related field.
• Experience working with low-income or homeless populations.
• Fluent in Spanish preferred.

Responsibilities

• Provide direct case management services on-site and in the field, including mental health and substance use assessments and service planning.
• Develop and update Individual Service Plans and document progress toward goals.
• Coordinate referrals to medical, mental health, and other providers; liaise with community organizations.
• Maintain tenant files and use electronic health records, ensuring accuracy and confidentiality.
• Facilitate life-skills groups and support services to promote housing stability.
• Collaborate with Workforce Opportunities staff to assist tenants with employment goals.
• Ensure compliance with organizational and funder regulations and complete required documentation.
