Job summary

Jericho Project in New York, NY is seeking a Case Manager to provide residential and long‑term support to formerly homeless adults transitioning to permanent housing. You will carry a caseload of 20–25 clients, conduct regular meetings, coordinate with health and social service providers, and maintain thorough documentation.

Responsibilities include assessments, developing service plans, crisis intervention, and collaboration with workforce opportunities staff to support clients’ employment

Qualifications

• Bachelor’s degree in human services or related field.
• Strong organizational, writing, communication, and computer skills.
• Experience with low‑income or homeless populations preferred.
• Fluency in Spanish strongly preferred.

Responsibilities

• Provide direct case management services on-site and in the field, including assessments and service planning.
• Coordinate with health and social service providers to support tenants.
• Maintain accurate tenant files and documentation in the electronic health record.
• Develop and review Individual Service Plans and monitor progress toward goals.
• Assist tenants with accessing public benefits and employment-related resources.
• Conduct outreach, group activities, and aftercare contacts as needed.
