Job summary

Jericho Project is seeking a Case Manager to provide residential and long-term support to formerly homeless or at-risk adults as they transition into permanent housing. The role involves a caseload of 20–25 individuals, frequent contact with tenants, and coordination of services.

The position is based at Fordham Village (Bronx) with a focus on housing stability, employment goals, and comprehensive wraparound services, including mental health and addiction support.

Qualifications

• Bachelor's degree in human services or related field.
• Strong organizational, writing, communication, and computer skills.
• Spanish fluency strongly preferred.

Responsibilities

• Provide direct case management services on-site and in the field.
• Use evidence-based models such as Motivational Interviewing, CTI, and Harm Reduction.
• Maintain accurate tenant files and document case management activities.
• Coordinate and document services and prepare Individual Service Plans.
• Serve as liaison between tenants and community providers, and document outcomes.
• Facilitate life skills and support groups, and complete referrals to medical and mental health services.
• Collaborate with Workforce Opportunities Staff to assist tenant employment goals.
• Ensure compliance with organizational and funder regulations; manage aftercare contacts.
