Job summary

Jericho Project in New York is seeking a Tenant Service Coordinator for the Gun Hill Road 79-unit Permanent Supportive Housing program. You will provide case management and service coordination for adults with mental illness and substance use histories, delivering Housing First based support in the office and at residents' homes.

Qualifications

• Bachelor's degree in human services or related field required.
• Experience with homelessness, substance use, mental health and/or trauma settings preferred.
• Willing to work a weekend day.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Maintain a case load of high risk/high intensity tenants.
• Provide enhanced case management using Housing First principles in office and at residents' homes.
• Conduct assessments and program intakes for all residents.
• Ensure data accuracy and complete required documentation.
• Develop and update person-centered service plans every 6 months.
• Coordinate with psychiatry and manage appointment times for residents.
• Provide referrals and linkages to public benefits and housing supports.
• Assist with community engagement and aftercare services as needed.
• Support coordination with Jericho’s Workforce Opportunities and other departments.
