Job summary

The People Concern is seeking an ICMS Case Manager to support clients on the 1603-Housing Retention program. Onsite role in Los Angeles, you will manage a caseload, coordinate services, and assist with housing placement and retention using harm reduction and trauma-informed principles.

You will collaborate with LA County DMH/DHS and work to improve health and independent living skills, while maintaining confidentiality and client-centered practices in a busy environment.

Qualifications

• High School Diploma or GED required or equivalent.
• Valid CA driver’s license with reliable transportation.
• Experience with case management and client advocacy preferred.
• Ability to work in harm reduction and trauma-informed settings.

Responsibilities

• Maintain a caseload of approx. 20 high-acuity Clients for intensive case management.
• Collaborate with DMH and DHS programs to coordinate treatment teams.
• Screen, enroll, and orient Clients to program policies and goals.
• Complete biopsychosocial assessments to guide treatment planning.
• Develop quarterly goals to improve quality of life and health outcomes.
• Maintain accurate progress notes in electronic data systems.
• Coordinate appointments and transportation to health services.
• Assist Clients with benefits applications and housing placement.
• Work with property managers and third parties to support housing retention.
