Job summary

Shelter House, Inc. is seeking a Case Manager in Fairfax, VA to provide trauma-informed, comprehensive case management for individuals and families experiencing or at risk of homelessness.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Development, or related field or commensurate.
• Three years of experience in service delivery with low- and moderate-income families.
• Ability to prioritize competing priorities and make sound judgments.
• Strong written communication skills.
• Valid Driver's License.
• Willingness to work evenings and weekends as needed.

Responsibilities

• Conduct comprehensive client assessments including housing history, barriers to housing, health, employment, education, financial stability, strengths and support needs.
• Manage an active caseload of approximately 18–20 households with client-centered services.
• Develop, implement and monitor Housing Sustainability Plans for long-term stability.
• Connect clients with community resources, benefits, healthcare, employment and education programs.
• Assist clients with budgeting and financial literacy.
• Maintain documentation in HMIS and client files per agency policies.
• Prepare reports and submit documentation timely.
• Collaborate with Shelter House staff and external partners.
• Uphold iC.A.R.E. values in all interactions.
