Job summary

Shelter House in Fairfax, VA is seeking a Supportive Housing Case Manager to oversee a caseload of 10–20 households, applying a trauma-informed, housing-first approach to support stability and self-sufficiency.

You will conduct assessments, plan services, and guide clients to housing, employment, and resources, while coordinating with a multidisciplinary team. Requires a bachelor’s degree or related experience and strong communication skills.

Qualifications

• A bachelor's degree in human services/ related field or commensurate experience.
• 1+ years of case management/direct services/related experience.
• Strong written and oral communication skills.
• Ability to prioritize competing priorities and make sound judgments.
• Ability to complete tasks while navigating frequent interruptions.
• Ability to deliver creative, resourceful solutions to unique challenges.
• Strong organizational and time management skills.
• Valid Driver’s License and reliable transportation.
• Ability to Drive a 15 passenger van.
• Willingness to work evenings and weekends when needed.

Responsibilities

• Perform assessments to gauge client strengths and needs using a strengths-based approach.
• Develop Individualized Service Plans with clients.
• Assist clients in securing services and coordinate delivery of housing, employment, finances, and health resources.
• Provide crisis support and implement emergency procedures.
• Maintain client budgets and money management education; ensure rent/utilities payments.
• Document interactions and update HMIS; complete reassessments and reviews.
• Participate in on-call rotation and staff trainings.
