Shelter House, Inc. in Fairfax, VA seeks a Supportive Housing Case Manager to handle a caseload of 10–20 households, supporting housing first with trauma-informed care. You will assess needs, plan services, and guide clients toward self-sufficiency within the supportive housing program.

The role requires a bachelor’s in human services or related field (or commensurate experience) and 1+ year of CM experience, with strong communication and organizational skills. Evening/weekend work may be needed.

Formación

• Bachelor's degree in human services or related field or commensurate experience.
• 1+ years of case management/direct services experience.
• Strong written and oral communication skills.
• Ability to prioritize competing priorities and make sound judgments.
• Ability to complete tasks while navigating frequent interruptions.
• Creative, resourceful solutions to unique challenges.
• Strong organizational and time management skills.
• Valid Driver's License and reliable transportation.
• Ability to drive a 15-passenger van.
• Willingness to work evenings and weekends when needed.

Responsabilidades

• Perform assessments of clients' strengths, health, and needs.
• Determine supports needed to maintain stability and achieve goals with clients.
• Develop Individualized Service Plans.
• Assist clients in securing services and coordinating delivery (employment, housing, finances, mental health, etc.).
• Provide crisis support and emergency procedures as needed.
• Create and maintain budgets and money management education for clients; ensure timely rent/utilities payments.
• Encourage clients to meet objectives in their service plans.
• Support housing program completion and discharge planning.
• Maintain complete client records and HMIS documentation.
• Participate in on-call rotation and staff trainings.
• Ensure compliance with agency policies and collaborate with multidisciplinary teams.
• Disseminate relevant information to staff.
• Proficiently use Microsoft Office in day-to-day work.
• Safely operate Shelter House vehicles for client transportation.
