Job summary

Northern Virginia Family Services (NVFS) is hiring a Case Manager to join the Community Housing team. You will deliver individualized case management, assist clients in securing and maintaining housing, and link them to services that promote stability and independence.

Responsibilities include eligibility screening, housing plan development, and collaboration with landlords and community partners. Strong English, trauma-informed practice, and HMIS proficiency are essential.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or related field.
• At least 1 year of case management experience serving homelessness.
• Experience with HMIS and maintaining electronic case records.
• Knowledge of Housing First, trauma-informed care, and motivational interviewing.
• Oral and written English fluency; Spanish or other languages preferred.

Responsibilities

• Screen eligibility, conduct intake, and assess housing goals.
• Provide trauma-informed case management and housing stabilization planning.
• Connect clients with employment, health, benefits, and community resources.
• Monitor progress and adjust service plans to promote stability.
• Collaborate with landlords and housing providers; conduct home visits.
• Maintain HMIS documentation and program records.
