Job summary

HELP USA is seeking a Case Manager who will help clients experiencing homelessness identify root causes, coordinate with the service team to develop an independent living plan, and meet regularly to advance toward housing.

You will guide clients to employment, education, medical and mental health services, childcare and support for substance use issues, with a focus on compassionate, client-centered service.

Qualifications

• Bachelor's degree in social work or a related field OR equivalent experience and skills.
• Able to keep track of multiple priorities, completing tasks on time and keeping the appropriate people informed of the status.
• Great communication skills (written and oral).
• Want to help people experiencing homelessness.

Responsibilities

• Help clients identify the causes of homelessness and coordinate with the service team to develop an independent living plan.
• Meet with clients regularly to help them work their plan and find a home.
• Provide direction and referrals for employment, education, medical and mental health services, childcare, substance abuse, and more.
