Job summary

HELP USA is seeking a dedicated Case Manager to assist people experiencing homelessness in Bronx, NY, helping them identify causes of homelessness and coordinate with a service team to develop independent living plans and secure housing.

You will meet clients regularly to help them work their plan and access employment, education, medical/mental health services, childcare, and substance abuse support as needed. Strong communication and organizational skills are essential.

Qualifications

• Bachelor's degree in social work or related field or equivalent experience.
• Ability to track multiple priorities and inform stakeholders of status.
• Excellent written and oral communication skills.
• Genuine desire to help people experiencing homelessness.

Responsibilities

• Identify causes of homelessness for clients and coordinate with the service team to develop an independent living plan.
• Meet with clients regularly to help implement the plan and work toward finding housing.
• Provide direction and referrals for employment, education, medical and mental health services, childcare, and substance abuse support.
