Job summary

The People Concern is seeking a Case Manager for our 1603-Housing Retention program at two LA locations. This onsite role supports clients experiencing homelessness toward housing stability by applying harm-reduction, housing-first, and trauma-informed approaches.

You'll manage a caseload, coordinate services with DMH/DHS partners, document progress, and help clients access benefits and independent living skills to sustain permanent housing.

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred or equivalent experience.
• Experience working with homeless individuals, preferably with mental health or substance use considerations.
• Ability to operate within harm-reduction, client-centered frameworks and maintain confidentiality.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management services.
• Collaborate with LA County DMH and DHS programs to coordinate treatment and services.
• Assist in screening, intake, and enrollment; complete biopsychosocial assessments and treatment plans.
• Coordinate benefits, appointments, transportation, and follow-up to improve housing retention.
• Document progress notes and maintain client charts per agency guidelines.
• Engage clients to build independent living skills and community integration.
