Job summary

Inspire Together in Los Angeles is seeking a dedicated Case Manager to provide client-centered case management and serve as the primary contact for participants working toward housing stability and self-sufficiency.

You will help clients identify needs, develop goals, access community resources, and support them in education, employment, and well-being while maintaining confidentiality and accurate records.

Qualifications

• High school diploma or equivalent; some college or a degree in social work or related field preferred.
• Minimum of 1 year of experience in case management or related field.
• Experience working with individuals experiencing homelessness or barriers is preferred.
• Strong communication, organization, documentation, and problem-solving skills.
• Ability to pass background and fingerprint clearance checks.

Responsibilities

• Conduct client screenings, intakes, assessments, and initial problem-solving conversations.
• Develop individualized case plans based on each participant’s needs, strengths, and goals.
• Maintain regular communication and follow-up with assigned clients.
• Connect clients with housing, employment, education, healthcare, benefits, and other resources.
• Provide referrals and coordinate services with community partners and service providers.
• Advocate for clients with social service agencies and other systems when appropriate.
• Participate in case conferences and collaborate with supervisors and other program staff.
• Maintain accurate, timely, and confidential case notes and documentation.
