Job summary

Making Opportunity Count is seeking a Shelter Case Manager to provide comprehensive case management for families in shelter, using motivational interviewing and strength-based approaches to develop housing plans that promote self-sufficiency and permanent housing.

The role involves coordinating with the Stabilization Program, ensuring timely documentation in ETO, and advocating for barriers to housing to be addressed through supportive services.

Qualifications

• Associate's degree in human services or related field.
• Minimum 1 year of case management experience focused on pre-tenancy and tenancy-sustaining services.
• Empathetic to diverse populations; strong interpersonal and communication skills.
• Ability to manage multiple cases and prioritize workload; budgeting skills.

Responsibilities

• Develop individualized rehousing plans with clear goals and measurable steps.
• Establish recurring monthly rehousing meetings with families.
• Document case management activities in ETO within 24 hours of contact.
• Review barriers to housing (credit, records, finances) and address them.
• Coordinate with Stabilization program and department staff for continuity of services.
• Provide referrals and connect families with community resources.
• Produce weekly housing placement and program reports; advocate for accommodations when needed.
