Job summary

Coalition for Responsible Community Development is seeking an ICMS Case Manager to deliver on-site case management for formerly homeless individuals at a new permanent supportive housing complex. You will link clients to health, education, and job services while guiding them through ISP development and stability plans.

The role requires coordinating with multiple agencies, maintaining client files, and reporting progress monthly; a driver’s license is needed for client transport and community

Qualifications

• 1-2 years of experience or related work and BA Degree.
• Experience working with youth experiencing homelessness and housing instability is beneficial.
• Reliable, flexible team player who works well with minimal supervision.
• Valid driver's license and car insurance as required by law.
• Proficient in Microsoft Office suite required.

Responsibilities

• Conduct initial housing intake and needs assessment with tenants.
• Develop Individual Service Plans (ISP) in collaboration with tenants.
• Facilitate housing processes and help tenants meet goals timely.
• Engage tenants via at least two face-to-face contacts monthly.
• Coordinate with property management and providers to ensure tenancy stability.
• Link clients to life skills, health services, counseling, education, and job training.
