Qualifications

• Experience in social services, counseling, crisis intervention, family services or housing services preferred.
• Bilingual language ability highly desirable.

Responsibilities

• Provide direct case management services including assessment, counseling, crisis intervention, referral, and advocacy to support relocation to permanent housing.
• Utilize DHS CARES and related systems to complete intake assessments and service plans.
• Advocate to ensure clients receive benefits, entitlements and services to aid transition to permanent housing.
• Coordinate services to stabilize families and improve life skills for self-sufficiency and housing stability.
• Document progress and ensure compliance with State, City and Agency regulations.
