Job summary

Qualifications

• Education: Bachelor’s degree preferred and/or an Associate degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice or Human Services.
• Experience: minimum one year in social services, counseling, crisis intervention, family services or housing services preferred.
• Bilingual ability is highly desirable.
• Excellent written and verbal communication skills.
• Excellent interpersonal and computer literacy in Microsoft Office.

Responsibilities

• Provide direct case management services including assessment, counseling, crisis intervention, referral, and advocacy to enable relocation to permanent housing.
• Coordinate services to stabilize the family unit and address substance use, housekeeping, parenting, and money management to achieve permanent housing.
• Maintain progress notes, service plans, and documentation for regulatory compliance and high-quality service delivery.
• Assist in transferring clients in and out of the program and participate in assigned on-call duties.
