Responsibilities

• Coordinates services to provide program participants with tools to sustain housing and prevent homelessness.
• Works with clients, households, supervisors, and staff to execute the service plan.
• Sets goals and actions to address immediate housing crises and underlying causes.
• Monitors and evaluates progress on service plan implementation.
• Convenes client cases with Supervisor and Program Director to determine subsidy needs.
• Prepares documentation for in-house subsidy applications for clients.
• Provides referrals to access community supports and entitlements.
• Maintains accurate records via case files and databases.
• Coordinates and delivers services to Homebase program participants.
• Ensures participants' rights are protected.
• Performs other duties as assigned by Supervisors or Program Director.
