Job summary

The Children's Rescue Fund in New York, NY seeks a Case Manager to provide direct case management, assessment, counseling, crisis intervention, and advocacy to help families transition to permanent housing.

You will coordinate services, maintain documentation in CARES, and collaborate with DHS and other agencies. Bachelor’s or Associate in social services preferred; bilingual skills desirable.

Qualifications

• Education and experience in social services, counseling, crisis intervention or housing services.
• Bachelor's preferred; Associate considered with relevant experience.
• Excellent written and verbal communication; strong interpersonal skills.

Responsibilities

• Provide direct case management, including assessment, counseling, crisis intervention, referral and advocacy.
• Coordinate services to stabilize families and support permanent housing transition.
• Document progress and maintain records; ensure timely data entry in DHS CARES system.
• Provide client advocacy to access benefits and services; maintain professional boundaries.
