Job summary

The Family Tree, Inc. is seeking a Homelessness Program Case Manager to deliver home-based case management to families and individuals facing homelessness or at risk.

This role uses evidence-based practices to identify barriers and help clients maintain housing stability through rental assistance, supportive services, and goal setting. This full-time position involves visiting homes, coordinating transportation, and flexible hours including occasional evenings or weekends.

Qualifications

• Must have experience working with under-resourced families and individuals.
• Trauma-informed, client-centered care with cultural competence.
• Strong data input skills and attention to detail.
• Ability to respond calmly to crises and de-escalate when needed.
• Self-directed work ethic with strong teamwork and organizational skills.

Responsibilities

• Provide and document home-based case management per program guidelines.
• Assist participants with housing navigation, benefits, referrals, and goal setting.
• Develop and monitor housing stabilization plans with clients.
• Assess eligibility for homelessness programs and document in HMIS.
• Enter client data accurately and support monthly grant reporting.
