Job summary

The Salvation Army is seeking a Full-time Case Manager to provide housing services support for individuals and families in Davenport, IA. Qualifications

• High school diploma or equivalent; two years’ prior case work or related experience.
• College coursework in human services preferred.
• Strong communication, organization and data entry skills.

Responsibilities

• Conducts client interviews regarding needs for assistance.
• Completes documentation and maintains client files (intake, inventories, service plans, case notes).
• Maintains documentation in HMIS/SIMS and other platforms.
• Educates clients about community resources and referrals.
• Provides financial literacy/budget counseling and referrals for supportive services.
• Processes paperwork for eligible participants per funding source requirements.
• Acts as advocate for clients within the interdisciplinary team.
• Follow-up after program completion as required.
