Job summary

This Stabilization Case Manager is a full-time, community-based position, providing ongoing case management support to families moving from homelessness to housing in Plymouth, MA. A Bachelor's Degree or equivalent, CPR, and related certifications are required.

You will work closely with families for a one-year term, making referrals to educational and employment opportunities and coordinating care to maintain permanent housing.

Qualifications

• Bachelor's Degree or equivalent work experience.
• CPR certification is required (provided by FBMS).
• AED and First Aid may also be required (provided by FBMS).

Responsibilities

• Complete intake process within required time frame for all family members 18+, utilizing a housing first framework.
• Collect and maintain data-based statistical information, maintain accurate case records with data entry as determined by program funders.
• Meet with participants quarterly to monitor progress and implement individual service plans for all family members 18+.
• Set up and maintain accurate case files and adhere to requirements determined by program funders.
• Provide outreach and assessment in the community as deemed necessary by supervisor.
• Collaborate with staff across housing search, child services, workforce development and prevention staff.
• Complete home visits and assist participants to access community resources.
• Participate in supervision, agency and community meetings and trainings.
• Initiate crisis intervention when necessary.
• Demonstrate professionalism, empathy, communication, and reliability in service delivery.
